Zerifax (Rifaximin) is a broad-spectrum antibiotic designed to combat certain bacterial infections. It works by targeting and inhibiting bacterial RNA synthesis, specifically effective against non-invasive strains of Escherichia coli. This action is crucial in managing conditions like traveler’s diarrhea and hepatic encephalopathy by reducing harmful bacterial neurotoxins in the gut.

Purposes

Zerifax is primarily used to treat:

  • Traveler’s diarrhea caused by non-invasive E. coli strains.
  • Hepatic encephalopathy to manage symptoms by controlling bacterial toxins in the gut.

Warnings

  1. Clostridium difficile-Associated Diarrhea (CDAD): Use of Zerifax may lead to CDAD, which ranges from mild to severe. Monitor for symptoms of CDAD, especially if diarrhea persists post-treatment. If CDAD is suspected, discontinuation of Zerifax may be necessary.

  2. Breastfeeding: It is unknown if Zerifax is excreted in breast milk. Consider the potential risks to a nursing infant when deciding whether to continue breastfeeding or using Zerifax.

  3. Hepatic Insufficiency: Caution is advised when using Zerifax in patients with severe hepatic insufficiency due to increased systemic exposure.

Adverse Effects

Common side effects include:

  • Traveler’s Diarrhea: Flatulence, headache, abdominal pain, nausea, constipation, and vomiting.
  • Hepatic Encephalopathy: Peripheral edema, nausea, dizziness, fatigue, and rash.

Uncommon effects may include severe reactions such as confusion, severe abdominal pain, and significant changes in mood or behavior. If any severe side effects are experienced, seek medical attention promptly.

Cautions

  • Discontinue Zerifax if symptoms persist for more than 24-48 hours or if diarrhea worsens, as it may be caused by pathogens other than E. coli. Alternative antibiotic treatments should be considered.
  • Always consult your healthcare provider regarding the use of Zerifax if you have existing medical conditions, particularly hepatic issues or if you are pregnant or breastfeeding.
