Zerifax contains 550 mg of Rifaximin, a potent antibiotic with a unique mechanism of action. It works by binding to the beta-subunit of bacterial DNA-dependent RNA polymerase, thereby inhibiting bacterial RNA synthesis. Rifaximin is particularly effective against non-invasive strains of Escherichia coli (enterotoxigenic and enteroaggregative strains). It also plays a role in reducing the production and absorption of bacterial-derived neurotoxins, such as ammonia, which are implicated in hepatic encephalopathy.

Purposes:

  • Bacterial Infections: Used primarily for treating bacterial infections, including those caused by non-invasive strains of Escherichia coli.
  • Hepatic Encephalopathy: Helps manage symptoms related to hepatic encephalopathy by reducing neurotoxic bacterial byproducts.

Warnings:

  1. Clostridium difficile-Associated Diarrhea (CDAD): The use of Zerifax may lead to CDAD, which can range from mild to severe and potentially fatal. If severe diarrhea occurs, especially after antibiotic use, consider discontinuing the drug and seek medical advice.
  2. Pregnancy and Lactation: The safety of Zerifax during pregnancy is not established. It should be avoided by pregnant women or those who are breastfeeding unless absolutely necessary. Consult your healthcare provider for alternatives or guidance on whether to continue breastfeeding.
  3. Hepatic Insufficiency: Use with caution in patients with severe liver impairment, as systemic exposure to Rifaximin may be increased in such conditions.

Adverse Effects:

Common side effects associated with Zerifax include:

  • Traveler’s Diarrhea: Flatulence, headache, abdominal pain, nausea, constipation, and vomiting.
  • Hepatic Encephalopathy: Peripheral edema, dizziness, fatigue, abdominal pain, and rash.

Less common but serious effects may include:

  • Diarrhea: Watery or bloody stools, which may indicate a more serious underlying condition.
  • Neuropsychiatric Symptoms: Such as confusion, hallucinations, or memory impairment.
  • Muscle and Joint Issues: Including muscle spasms, arthralgia, and myalgia.

Cautions:

  • Diarrhea Management: If diarrhea persists beyond 24-48 hours or worsens, particularly if caused by pathogens other than Escherichia coli, consider discontinuing Zerifax and evaluating alternative treatments.
  • Drug Interactions: Be aware of potential interactions with CYP3A4 substrates and other rifamycins, as these may influence the effectiveness of Zerifax.

  • Administration: Take Zerifax tablets whole, without crushing or chewing, to ensure proper release and absorption of the medication.
